School Immunization Assessment Report Summary

All schools in Hawaii are required to report students who have not complied with the immunization requirements upon school entry and students who have medical or religious exemptions. A final report is due to the Department of Health by January 10th of each school year.
The information displayed in the map is self-reported medical exemption, religious exemption, and incomplete exemption* data submitted by each school for students in kindergarten through grade 12. Self-reported data are not validated by the Department of Health. Schools that have not submitted the final report are noted as "Did not report".
