University of Hawaii: COVID-19 Community Risk Survey

The Pacific Urban Resilience Lab (PURL) and the National Disaster Preparedness Training Center (ndptc.hawaii.edu) at the University of Hawaii, supported by the U.S. Federal Emergency Management Agency (FEMA) and community partners, conducts research and develops training to increase community resilience. We are conducting this brief survey on attitudes towards protective actions for the COVID-19 pandemic. The information from this survey will help to inform community mitigation strategies for the disease. The survey will take 10 minutes to complete.
Benefits and Risks: The survey will help us better understand how COVID-19 has spread and the willingness to take protective actions. There are no direct benefits to participants for taking this survey.
Voluntary Participation: Your participation in this project is voluntary. If you prefer not to answer a question, you may skip that question. If you choose not to complete the survey, you may stop at any time.
Privacy and Confidentiality: At the end of the survey, you will be asked to provide your age, gender, email, and phone number. This information is strictly voluntary, is not required, and will be kept strictly confidential. Background questions such as age, ethnicity, and occupation are included to compare respondents to census distributions to determine the survey's representativeness. Individual responses will be kept confidential in a secured database and will not be released publicly. Data will be made available only to the research team and authorized agencies, including the University of Hawai'i Human Studies Program. Summary data will be used in reports and publications and shared with the participating organizations and individuals upon request.
